Help me find this debug monitor in my files please

im having this problem

I DID NOT install any debug monitor script on my server
I am using the admin tools with no debug option

whenever someone presses Insert on the keyboard a debug window pops up for 20-30 seconds that looks like in the screenshot provided

can anyone tell me where it is located so i could remove/edit it?
